Value and the Medical Home: Effects of Transformed Primary Care
Our advanced patient-centered medical home model enhances health and wellness for complex ambulatory patients, while also reducing use.
Our ProvenHealth Navigator model focuses on population stratification, early identification of risk factors, compliance with practice guidelines, medication adherence, close patient monitoring, and enhanced access to outpatient care. Use of electronic devices and telemonitoring protocols for high-risk patients with certain diseases to support timely communication between patients and providers is a valuable component of the model.
ProvenHealth Navigator has had a substantial impact on population health, reducing, for example, 30-day hospital readmission rates for patients with congestive heart failure by more than 44%.
