Ten years ago, Geisinger introduced the ProvenCare model, a multicomponent program proven to facilitate delivery of reliable (every patient, every time) evidence-based care.
This approach was first conceptualized by Dr. Glenn D. Steele, Jr., Geisinger’s CEO from 2001–2015, who now serves as chair of xG Health. The model has reduced mortality, complications, length of stays, and readmissions while increasing system efficiency and profitability.
Our ProvenCare model is based on working with clinicians to build buy-in to best practices, which reduces variation in care, then redesigning workflow to incorporate these best practices, followed by integrating workflow redesign into the electronic health record. In addition to increasing efficiency and compliance with best practices, this enables real-time decision support.
The ProvenCare portfolio, recognized by top healthcare experts and the international media, includes ProvenCare Acute, which focuses on delivering best practices for acute episodic care in the inpatient setting, and ProvenCare Chronic and Prevention, which facilitates delivering evidence-based care for patients in the outpatient/ambulatory setting.
