You’re a healthcare provider in one of the four regions included in CPC+ “Round 2.” You want to be a part of this exciting program that, in Round 1, has already begun to demonstrate positive results.
First, keep in mind these important dates:
Applications to participate in Round 2 are due July 13, 2017.
Round 2 will begin in January 2018.
You’ve likely already begun preparing your application — and if so, congratulations for getting involved! If you’re still unsure, we strongly urge you to apply to be part of this grand undertaking.
CPC+ is the first CMMI program to provide upfront funding to support investments in primary care practices to build the team and tools necessary to keep their patients healthy. The application process and the implementation itself both represent opportunities to build a sustainable care model to truly manage your patient’s health — and to strengthen programs you’ve already begun to roll out.
While the window to apply for CPC+ Round 2 is small, there’s still time to get on board. By working with a leader in population health and care model transformation, you can accelerate your preparation and implementation, and begin to achieve results reliably and consistently.
With the insights gained while preparing your application, you will be well-positioned to:
Better understand your population empanelment, data and analytics
Develop and implement an improved risk-stratification process, including your ability to flag complex care patients for care management
Recruit and build teams, and develop care plans, that support your care management model
Develop practice site top-of-team roles and training
Expand access and add 24/7 capability
